How can we identify that our friend has mental illness?

How can we identify that our friend has mental illness?

My husband and I have this good friend. He is carefree, wonderful fellow and fun to hang out with. what only disturbs us is his usually odd behavior. He voluntarily tells us that he usually talk to himself in the mirror, do his monologue, and just act like his going nuts. He is fully aware of his actions and he told us that the things that he did is his way of passing time and entertaining his mind.

He doesn't have any girlfriends yet (not that we know of) and I am a bit worry about his mental health.
